Control Group is retrospective data, 2001. Care Kit Group is one quarter, 2002. Selection criteria: patients discharged to home or home care weighing 300 pounds or less. Data collected from medical records. Readmission DRGs include 014, 015, 023, 073, 079, 088, 099, 090, 107, 115, 116, 120, 121, 124, 128, 130, 138, 141, 143, 145, 182, 183, 294, 296, 316, 403, and 468. |
HIGHLIGHTS: Patients may be readmitted with CHF symptoms and be treated as CHF patients, but get a code that is different than DRG 127. For the first time, this hospital looked at all CHF-related readmissions and discovered that the readmission rate was much higher than the rate for 127 alone. |
